CheckIn - Student Tech Support

Waiver: I agree by asking the University of New Haven Office of Information Technology (OIT) to repair my personal computer and/or device that I will not hold them responsible for any damage to my device(s) or loss of data to my equipment. I accept all responsibility to back up my data and understand that loss of data is a potential risk by the service I am requesting. I agree to pick up my equipment within 60 days of notification and I understand if I do not collect my equipment in this time-frame OIT may dispose of my equipment and accessories. I understand my equipment may only be returned to me and I must provide a photo ID at the time of pickup. By submitting this form, emailing, having OIT fill this form out on my behalf, and/or dropping my device off for repair to OIT, it is recognized as my (the student requesting the repair’s) legally binding electronic signature, and I accept all the above terms.
